How does a person get referred to RHSJ?
What happens once a person is accepted into
a treatment program?
THE PROCESS
The referral process is set in motion when the patient’s
physician, case manager or discharge planner requests
an evaluation from us. This can be done simply by
calling (856) 696-7100 to arrange for clinical assessment
by an RHSJ liaison. The individual will facilitate
transition between the acute care setting and the
rehabilitation hospital setting. Information helpful
to the liaison includes patient diagnosis, medical
stability, endurance, and motivation. The liaison
not only assesses the patient’s condition
but actually starts the plan of care. We believe
that this approach assures that many good things
happen, the most important of which that the patient’s
transition to our setting is a smooth and stable
one. Using a liaison to assess patient needs before
the patient reaches our doors gives us utmost confidence
that the patient will have a productive, uninterrupted
rehabilitation stay.
THE TREATMENT AND TRAINING
The individualized education and therapy program
is developed in the patient’s care plan. We
assure continuity and integration with family physicians,
specialists, and the family involved with the patient’s
care.
THE REFERRAL AND PLACEMENT
Once active rehabilitation has ended, the patient
is transferred to a program of care where progress
can continue. To be sure that we are focused on
community reintegration, we will want to know where
the patient intends to live after the comprehensive
rehabilitation programs. Appropriate follow-up with
the family physician in encouraged and facilitated.
Patients admitted to the Rehabilitation Hospital
of South Jersey are encouraged to wear street clothes
and have their meals in the dining room. Special
requests for meals are honored by our dietary staff.
Family may visit between the hours of 8:00 am and
8:00 pm, and may arrange to have meals with patients.
Additionally, family representatives are encouraged
to participate in clinical team meetings.
